Successful endodontic therapy depends largely on effective root canal disinfection. Despite advances in instrumentation, complete bacterial elimination remains challenging due to the complexity of the root canal system, including lateral canals, isthmuses, and dentinal tubules. This lecture will explore contemporary disinfection protocols integrating modern irrigants, activation techniques, and adjunctive technologies. Emphasis will be placed on sodium hypochlorite optimization, chelation strategies (EDTA, HEDP), ultrasonic and sonic activation, negative pressure systems, intracanal medicaments, and emerging approaches such as photoactivated disinfection. Clinical recommendations will be supported by recent literature and practical workflow strategies to improve predictability and long-term treatment success.
