
Prevention and endoscopic management of intra-operatory and post-operatory complications in lateral access sinus lift procedures
Due to the bone atrophy and pneumatization of the maxillary sinus, inadequate bone height in the posterior maxilla can complicate dental implant treatment. Many techniques have been described in the literature to elevate the floor of the maxillary sinus and allow dental implant placement. Although different safe and effective sinus lifting techniques have been well-documented for almost 50 years, numerous complications and comorbidities still encountered during clinical practice. In this lecture, the parameters that directly influence the selection of sinus lifting technique will be explained in detail through endoscopic videos showing the underlying biology and sinus function. Additionally, the risk factors for maxillary sinus complications will be addressed and when preoperative otolaryngologic evaluation and advanced imaging of the sinuses is necessary for sinus lift candidates will be enlightened. Bone atrophy combined with maxillary sinus pneumatization often results in insufficient bone volume in the posterior maxilla, making implant rehabilitation challenging. Over the past five decades, numerous sinus floor elevation techniques have been developed to facilitate predictable implant placement. Despite the long-standing success and safety of these procedures, intraoperative and postoperative complications continue to be encountered in daily clinical practice.
This lecture will explore the key parameters that guide the selection of the most appropriate sinus augmentation technique, supported by endoscopic videos that illustrate the underlying biology, sinus physiology, and the impact of surgical interventions on sinus function. Particular emphasis will be placed on identifying patient- and procedure-related risk factors for maxillary sinus complications, as well as defining when advanced radiographic evaluation and preoperative otolaryngologic assessment are indicated.
Finally, the lecture will present a practical, multidisciplinary approach to the diagnosis and management of maxillary sinus complications. Through real clinical cases and endoscopic documentation, participants will gain evidence-based strategies for preventing complications, recognizing them at an early stage, and selecting the most appropriate treatment in collaboration with otolaryngologists.
